Please keep a copy of the application for your records and then email it to us. You may also fax them to 954-337-6164. All forms can be completed in Adobe Acrobat (not in Adobe Reader). If you do not have adobe, you may print them to complete.

CFH Information Checklist - Checklist of all necessary documentation
CFH Business Loan Application - To be completed
CFH Business Plan Worksheet
CFH Business Projections Worksheet
CFH Environmental Questionnaire - Complete if Real Estate is involved
CFH Management Resume - Complete for All Active Principals (or submit original resume)
CFH Personal Financial Statement - Complete for all owners and/or guarantors of 20% or greater (Spouses must sign also)
CFH Notes Payable Stocks and Bonds - Complete for all owners and/or guarantors of 20% or greater (Spouses must sign also)
CFH Schedule of Real Estate Owned - Complete for all owners and/or guarantors of 20% or greater (Spouses must sign also)
Credit Report with 3 scores and 3 years personal and business tax returns must be submitted for preliminary commitments.
